[Accident Report] JASDF F-15 accidents in 1983 to 1995 in Japan

has a brief list of JASDF F-15J (one seat) and F-15DJ (two seats) accidents and crashes, from 1983 to 1995, in Japan.

22 November 1995:
F-15J. 52-8846.
308th Hikoutai.
One pilot ejected and recovered.
About 165 km north-northeast of Komatsu AB, during ACM training, practising simulated AIM-9 firing. Wingman (62-8870) accidentally fired a missile and hit its tail.
Pilot bailed out and recovered by a fishing boat.
Aircrft raised, investigated, and scrapped.

6 October 1995:
F-15J. 72-8891.
303rd Hikoutai.
One pilot not wounded.
At Komatsu AB, while taking off for night air defense combat training, had trouble, hit the crash barrier at the end of the R/W06 runway, and stopped after 5 metres overrun.
Caught fire, but extinguished after some minutes.
Pilot got out by himself.

6 October 1993:
F-15DJ. 82-8064.
202nd Hikoukyoudoutai (Aggressors Squadron).
Two crew ejected and recovered.
In Hokkaidou, about 20 km south of Tomakomai City, above Pacific Ocean, had fuel system problem and crashed.
Between discovery of problem and bail out, the crew investigated the problem for about 20 minutes.

27 October 1992:
F-15J. 72-8884.
204th Hikoutai.
One pilot killed.
At 15:25, crashed in the sea 55 km from Kitaibaraki City, Ibaraki Prefecture, while returning to Hyakuri AB, after an air combat training sortie.
Pilot announced uncontrollable and bailed out. Recovered by a fishing boat and taken by a SAR helicopter, but died.

13 December 1991:
F-15DJ. 12-8079.
201st Hikoutai.
One pilot wounded.
Took off from Chitose AB.
Preparing to land at Komatsu AB, when had an explosion in rear of aircraft.
At 10:01, crashed in the Japan Sea, northwest of Komatsu AB.
Pilot bailed out at height 600 m.

2 July 1990:
F-15J. 52-8857.
204th Hiktoutai.
One pilot killed.
Practising 2 vs 2 radar intercept combat training above Kashimanada Sea, 70 km east of Hyakuri AB.
At 19:24, while descending from 10,000 feet to 5,000 feet, disappeared from radar and crashed.

29 June 1988:
Two F-15J. 22-8804 and 22-8808.
303rd Hikoutai.
Two pilots killed.
At 16:18, during air combat training, while evasive manoeuvring, the number 2 in the defensive flight of two, and the number 3 in the offensive flight, collided, caught fire, and crashed in the sea near Komatsu.

13 March 1987:
F-15J. 42-8840.
204th Hikoutai.
One pilot killed.
During air combat training, number 1 in a flight of three became vertigo, and crashed in the sea 130 km east-northeast of Hyakuri AB.

20 October 1983:
F-15DJ. 12-8053.
202nd Hikoutai.
Two crew killed.
At 18:32, during low altitude night training, crashed in the sea near Miyazaki Prefecture.
