Systemic failures at Irish flying school in fatal crash

Hi, I usually populate the Historic forum but popped in to share this with you.

It concerns a double-fatal helocrash that killed a pupil and instructor.

Would this be tolerated in Britain? I don’t think so. And I live in Ireland.
