More than just one failure……..
The AAIB report makes some very interesting reading to the technically minded (way above the level of the Press) and highlights some very important issues relating to basic / initial design, maintenance, crew operation and air traffic control issues. The fact that the aircraft in question actually reached its destination (Bristol) AND landed safely, can only be put down to pure luck.
To anyone with any real aviation connections I say, download the report, read it carefully and see what you can do to prevent such a set of occurances happening again. It is truly applicable to a lot of us.
And for my part, well I am a systems design engineer, and I still have a lot to learn, even after 50 years. I also know that I will never know it all but am more than willing to learn on a daily basis.
