Upper LH and RH engine bearer retaining bolts did not have any nuts installed.
Investigation of the aircraft records determined that the last access was in 2009 to replace some pulled rivets. It is assumed that after the replacement of the rivets the subject nuts were omitted to be refitted.
Eek, nearly missed seeing that one.

I read it as they were already fitted, but the nuts were removed for access to sort out a couple of rivets, then they forgot to put the nuts back on.

Given that the nuts are used to both fit and remove the taper pin, (threaded both ends) one can only presume that either the pins were fitted correctly with the nuts, and then the nuts were removed….or.. the pins were hammered in…..
