There were 2 separate problems…
The failure of the main parachute to open was identified early on and published by the CAA…

The cause of the seat firing has not AFAIK been openly published yet.

Unlike Commercial Air accident investigations. The MOD Don’t usually publish their findings to the media. In my own opinion this was a tragic accident created by a mishap, possibly failure to disarm the ejection seat before and after flight operation’s. Usually by the insertion of the disarming pins after use which prevents the operation of the ejection seat handles.

As David posted…no mods have been carried out to the hawk T1 seats since the accident,a couple of procedures/checks have been changed slightly….but that is the normal evolution of a/c checks etc !

Many thanks David. As I suspected, as presumably there are many of the seats in service and have been for some time. Perhaps an unfortunate term to use although it is difficult to imagine that a QC was not aware of the implications. Presumably the MOD report is not public?

Paul -this is a pre inquest hearing. The Mod I believe completed its report circa June. It appears to be a lawyer using the term ‘design fault’ -the seats are still in use since the incident and I don’t believe any modifications have been carried out .
