Hurricane Data Plate

Hello gents!

I’m looking for some help with a Canadian made Hurricane data plate. Mine is from RCAF5467 but is badly bend and twisted. It’s difficult to make out some of the wording because of the twists. Can anyone tell me what the plate should say. It’s appears to be quite different from the British made plates.
