Civil Aeronautics Board
Aircraft Accident Report
http://www.baesel.net/capaccidents/1958.04.06_RevisedCapitalAirlines_Viscount.pdf

‘This crash is interesting in that it was originally attributed to pilot error. A few years later a similar fatal incident occurred (CO290 29jan63 MAF-MKC) and the CAB revised their findings (1965). Buildup of undetected (horn type) ice on the horizontal stabilizer caused the aircraft to pitchover and thus created an uncontrollable flying condition.’
