TFC P38 info needed..

I’m taking an Aviation Maintenance Ethics course at the local community college & one of the things we have to come up with is an accident report. I picked the TFC P38 incident & tried to find any CAA reports this morning without any luck at all. Anyone have a link to the accident report? (or any other pertinent info)
