From the AM F.1180 accident card:
Pilot: F/O. W.R. Cook (11375) (NZ).
Duty: Operational patrol.
Time: 21:00 hrs.
Description: “Aircraft hit by friendly AA. fire. Out of control with sparks issuing from port engine – crew abandoned.
A/c chasing hostile aircraft and it would appear that the gunners mistook this A/c. for target.”

No indication of any injuries to crew. Both engines and aircraft Cat. “E”.

If you would like a jpg of the record card for additional details I could try a photograph of the reader screen. Send P.M.
