If I remember it correctly the flight num was Swissair 111 heavy and the pilot was a Captian Zimmerman.

On one of the aircraft investigation programs i belive it was the aircrafts entertainment system which caused a cockpit fire, cannot for the life of me remember the aircraft/date. I seem to remember that the E/S was the only thing that could not be switched off completely and as it was wired through the cockpit and into the cabin the wires overheated. Anyone shed anymore light on this?

the actual cause was the improper installation of the wiring for said Entertainment system. Pictures taken from a concerned employee and the wiring from other MD-11s in the fleet with the same system showed that several rubber gators that protected the wires from rubbing against metal joists were missing.
The wires rubbed, became exposed, shorted… and smouldered. IT was at this point the insulation came into the game and cause in uncontainable fire. The smoke from which most likely incapacitated the crew.

Sad sad event…. totaly avoidable

Was dry-ice not mistaken for smoke once?

Better safe than sorry, but what was the dry-ice being used for?

Smoke in the cockpit or cabin seems to be a relatively common occourance in airliners. Any ideas what is the actual cause of this?
