ejection seat accident

Does anyone knows of accidents involving ejection seats ie that technicians fixing or maintaining ejection seats accidentaly set them off? Have anyone survive such ejections? How could someone be blown up together with the seat when the person is not strapped into it?
If the seat fired accidentaly will the Miniature Detonating Cord on the canopy be fired too? MDC is usually fitted to British jets ie Hawk and the Harrier. Some new Russian jets is also fitted with these devices.
FYI, a Malaysian AF tehnician died after he was ejected on the ejection seat of a Hawk 108 on March 18, while demonstrating the use of the seat to newbies techs. He died on the spot while the six newbies were injured.
