Missing man formation

Hello all forum members,

Does anny one know what the real missing man formation looks like. I know there is one where one spot is open in a 4 ship formation and I know its a tradition in The Netherlands that in a 4 ship formation above the monument/grave ore nearby breaks formation and goes up in the sky as a resemble of the fallen pilot who goes to heaven.

can anyone tell me more about this formation?
