Could any pilots confirm if there exist any hold areas for gaining height (in restricted air space) ? If they exist, what are the procedures for gaining height in such cases – do they spiral up or do they attain height in increments ?

I know of nothing like this in any European Airspace.

It would be very unlikely for any aircraft to be given a height gaining hold. Normal procedure is for ATC to clear the aircraft to different levels consecutively whilst enroute.
There may be mountain airports that are an exception to this rule, but I am unaware of any.
