Engine malfunctioned, and crashed into mountain.
Pilot ejected, and rescued after two days.
5 December 1990:
ROCAF F-104J, 4511.
During ACM training, exceeded G limit, and disintegrated.
Pilot killed.
24 August 1990:
ROCAF TF-104G, 4188.
During landing, crash-landed.
Repaired, and scrapped in 1995.
Two pilots OK.
29 August 1968:
JASDF F-104J, 46-8604.
Heavily damaged in landing accident.
Front fuselage preserved at Kissa Hikoujou (Chimoto Coffee restaurant), in Hamamatsu city, Shizuoka Prefecture, as of 4 May 2006.
