1st Corps, National Airborne Service Corps, Ministry of Interior, ROC Government, based at Song Shan AP, Taipei.
After 10 AM, delivered six technicians (five Japanese and one Taiwanese) onto a civilian freighter (see below), to survey her oil pollution.
About 1:20 PM, came back, but just before retrieving the technicians, stalled, and crashed into sea.
By 2:30 PM, all five crew rescued.
Pilot, LIN Zhen Xing was critically wounded, or OHCA (Out of Hospital Cardiac Arrest).
By 6:30 PM, passed away, at hospital.
Crew member, CAI Zong Da, 35, was outside helo, and hoisting down to deck of freighter.
He was reportedly hit (head, body, and left leg) by rotor, and killed.
Others were lightly wounded.
Co-pilot, ZHU Yao Zhong, 46.
Flight engineer, XU Yi Yang, 35.
Crew member, SHI Ming Jie, 33.
Lin, 47, was retired Commander, ROCN, and flew S-70C.
Total flight time, as at 2016 February, 3,498 hours, of which 795 hours were AS365N.
Joined NASC in 2009.
Zhu, 46, was retired Lieutenant Colonel, ROCA.
Total flight time, 4,154 hours, of which 96 hours were AS365N.
Joined NASC in 2015.
Accident helo was manufactured in 1999.
Total flight time, 3,838.5 hours.
Engine 1, 426 hours.
Engine 2, 2,067 hours.
Last maintenance was on 1 March, after 300 flight hours.
According to witnesses who video-taped the accident, helo was between coast and freighter.
It flew south, and suddenly rotated clockwise.
After two rotations, it descended.
After another reverse rotation, when it faced the coast, and tail towards freighter, it crashed into sea levelly.
By 7 PM, an excavator recovered the wreck from beach to ground.
On previous day, 10 March, about 10 AM, 0.3 NM off Shi Men District, northern New Taipei City, the civilian freighter De Xiang Tai Bei drifted, ran aground, breached, and was flooded.
She had 21 crew: 16 Taiwanese and five foreigners.
NASC (and other) heloes scrambled for SAR.

Runway closed 4 minutes.
Civil passenger aircraft arrived, but landing delayed 30 minutes.
14:10, helicopter took off, at Akita AP.
After 20 minutes, 50 km south of AP, near Choukai Mountain, auto-pilot system malfunctioned, and co-pilot’s speed meter was zero.
Returned and landed at AP, 1 hour before schedule.
No person wounded.

Both pilots promoted one rank: Wang to Lieutenant Colonel, and Huang to Captain.
After ceremony, four AT-3 flew “missing man” formation; the fourth AT-3 climbed away from the formation.

During night training, about 3,500 m, engine shut down, and couldn’t restart.
Glided to 1,500 m, avoided two populated areas with lights, and crashed into mountain.
Aircraft carried 2.5 tons of fuel and 200 rounds of ammo.
Pilot ejected at 350 m, and lightly wounded (neck and waist).
Tower ordered him to eject at 1,000 m, but he dodged populated areas, and ejected when lower than 500 m.

2nd Hikoutai, 4th Taisensha Helicopter Tai, JGSDF, based at Kisaradu JGSDF AB, Chiba Prefecture.
16:30, a Leading Private [LPT] tasked with maintenance for that helicopter, was off-duty, but he opened its refuel cap, used a lighter to set fuel on fire, and partly damaged the helicopter.
Other soldiers discovered his action, and military police arrested him.
Leading Private was above 20 years old, and male.
He was reportedly upset by senior officer’s guidance instructions.
2014/06/09, he was charged with damage defense material, against SDF laws.
2015/09/25, he was dishonourable discharged.

1,489 hours 30 minutes.
Was ROCAF Thunder Tigers (similar to USAF Thunderbirds) member, now fighter instructor.
32 years old, and married.
Back seat, student: First Lieutenant HUANG Jun Rong.
95th Class (Year 103 = 2014), ROCAF Academy.
116 hours 45 minutes.
23 years old, and not married.
One of 10 trainers in the air at the same time.
11:55, took off, towards A6 area, that is, 67 NM northeast of AB.
12:25, above Ma Bo La Si Mountain, Nan Tou County, disappeared.
Altitude was above 3,000 m.
Location was Xin Yi Town, Nan Tou County, near Hua Lian County.
Last scheduled inspection of aircraft was 27 October 2014.

About 6:38 AM on 2015/09/26, a CH-47 discovered debris about 1.5 NM northwest of final location of radar blip.
By 12:50 noon, on ground, special ops soldiers found pilots’ bodies in cockpit of wreck.
The bodies will be recovered ASAFP, before Typhoon Du Juan arrives.

Wang’s father, WANG Rui Xing was also a ROC military (probably ROCAF) helicopter pilot.
When Wang was 10+ years old, senior Wang’s helicopter crashed at a farm, and rolled.
Senior Wang crawled out of a broken window, and survived.
Two years after this accident, Senior Wang voluntarily retired.

FWIW, Japanese Netters compared the SDF aircraft inventory as at 2015/03/31 and 2014/03/31, and WRT JASDF, noted 2015 has five fewer F-4EJ than 2014.
2014 had 60 F-4EJ and 13 RF-4E/EJ.
2015 has 55 F-4EJ and 13 RF-4E/EJ.
