JGSDF announced accident cause was fuel controller malfunctioned suddenly.
Inside engine, fuel controller injected fuel.
During maintenance test flight, pilots decreased engine output, then increased engine output, but engine did not restore output, and could not resume normal flight.
About 50 same type helicopters will replace with new fuel controllers, and resume flights from June end.

June 14-17, fuselage middle, right wing inner engine, and part of left wing recovered from 300 m seabed to barge.
June 19, barge arrived at JMSDF Iwakuni AB.
Flight recorder recovered from fuselage middle.
For recovery, aircraft detached as fuselage front, fuselage middle, and left and right wings.
JMSDF Iwakuni AB had five US-2 and two US-1A.
Four US-2 survivors grounded after accident.
3 June 2015:

13:25, ANA flight [B-737-800] to New Chitose AP, Hokkaidou, aborted take-off, as a JASDF CH-47 flew across and before ANA flight.
Although air traffic control instructed JTA [Japan Trans Ocean] flight [B-737-400] from New Ishigaki AP, Okinawa Prefecture, to abort landing, it landed, when ANA flight was on runway.
ANA flight 1694, call-sign All Nippon 1694, had 83 crew and passengers.
JTA flight 610 had 44 crew and passengers.
CH-47, call-sign Pony 41, had seven crew and passengers.
ANA flight was taking off on runway, from north to south.
CH-47 flew near runway south end, from east to west.
ANA flight stopped near middle of 3,000 m runway, had not moved to taxiway, when JTA flight landed on runway, from north to south.
JTA flight stopped 400 to 500 m behind ANA flight.
CH-47 claimed to misunderstand permission to take-off for ANA flight as for CH-47.
CH-47 pilot had 1,850 flight hours.
Co-pilot had 1,700 flight hours.
ANA pilot and CH-47 pilot confirmed permission to take-off to air traffic control, but both call-signs sounded alike, and air traffic control misunderstood who confirmed permission.
JTA flight claimed when received instruction to abort landing, it had landed, and engines were reverse-thrusting.

14:55, 40 km northeast of Ashizuri Cape, Kouchi Prefecture, during landing on sea and take off training, because tall waves, could not take off.
One of two right engines fell off, and damaged right float.
19 crew evacuated to life boat(s), and rescued by civilian tanker.
Four lightly wounded.
Primary pilot was Lieutenant Commander Shibata Shuuji (39).

On 11 March 2011, at Miyagi Prefecture, Matsushima AB, 18 F-2B of 21st Hikoutai, JASDF, were damaged.
All 18 were investigated, and 13 would be repaired.
21st Hikoutai had about 20 F-2B.
Including prototypes, 32 F-2B were produced.
F-2 production line already ended.
On 21 April 2015, at Aichi Prefecture, Mitsubishi Heavy Industries, Komaki Minami factory, the first repaired F-2B will be delivered to JASDF.
Repair cost is 13 billion Yen per aircraft.
Purchase cost was 11 billion Yen per aircraft.
The first repaired F-2B is one of six aircraft in fiscal year Heisei 27 [2015].

10:10, engine trouble, and emergency landed, at field, Okada, Miyagino Ward, Sendai City.
5 km east of JGSDF Kasuminome AB.
Field was 2 km from coast, flooded by tsunami on 2011/03/11, and abandoned.
During landing, main rotor blades bent.
Tail rotor and part of tail boom broke off, and landed 30 m from fuselage.
During test flight after periodic inspection.
9:44, took off, at JGSDF Kasuminome AB.
Two male crew not wounded.
Pilot: Major.
About 40 years old.
Mechanic: Sergeant [E-4].
About 30 years old.

67 crew and passengers: eight crew and 59 passengers.
11:00, at Tokushima Awaodori AP, during landing, co-pilot saw vehicle on runway, 1,000 to 1,500 m before aircraft, aborted landing, and went around.
Rear landing gear touched runway, at 250 km/h.
Vehicle was workers repairing landing lights.
Tokushima Awaodori AP is JMSDF Tokushima AB.
Tokushima Kyouiku Koukuugun, JMSDF, operated control tower.
Tower should have four staff.
During incident, only one staff, who forgot vehicle, and authorised aircraft to land.
Other three staff attending other tasks.
Runway length 2,500 m, width 45 m.
Second landing also aborted because turbulence.
Third landing successful, after 25 minutes.

VMFA-323, USMC, based at MCAS Iwakuni, Yamaguchi Prefecture, Japan.
Morning, northeast of Taiwan, two F/A-18s were en route from Japan to Singapore, for exercise Commando Sling, and escorting one EA-6B.
F/A-18 wingman had engine malfunction, requested emergency landing in Taiwan, and both F/A-18s diverted to Taiwan.
Two ROCAF F-16s scrambled from Hua Lien AB, and escorted the Yanks to Tai Nan AB, Tai Nan County.
About 1:19 PM, emergency landed at Tai Nan AB.

On 2 April, about 8:34 PM, an US military C-130 landed at Tai Nan AB.
Carried 10-20 ground crew and a spare F/A-18 engine.
C-130 had tail code “YJ” and “36 AS” = 36th Airlift Squadron, 374th Operations Group, 374th Airlift Wing, 5th Air Force, Pacific Air Force, USAF, based at Yokota AB, Toukyou, Japan.
The ground crew didn’t work overnight.
The next morning, they replaced the engine.
About 10:25 AM, F/A-18 nose number 404 departed its hangar.
About 11:31 AM, F/A-18 nose number 410 departed its hangar.
About noon, pre-flight checks.
About 1:13 PM, from runway 18R, took off.
About 3:15 PM, C-130 also took off.

ROCA, based at ROCA Gui Ren AB, Tai Nan City, Tai Nan County.
About 10 AM, at its home base, during training, while hovering at 100 cm (waist-high), then flying backwards for 50 m, collided with another TH-67 (208) parked on the ground.
Accident helo (212) damaged its skids, tail boom, and tail rotor.
Also damaged tail of 208.
Instructor pilot assumed control, and emergency landed.
Two pilots OK.
Instructor pilot: WEI Jian Hua.
Flight time: 8,693 hours.
Retired military pilot, and hired as contract instructor.
Student pilot: HUANG Guo Shu.
Flight time: 20 hours.
Both heloes are about 17 years old.
