Jun. 27, 2014 – 06:18PM JST
TOKYO – The U.S. military says that the cause of a U.S. Navy helicopter crash which occurred in December last year in Miura, Kanagawa Prefecture, was human error.
In a statement released to media, the U.S. said the accident was caused by inferior maintenance work on the helicopter, TBS reported Friday.
It said a clasp attaching the tail rotor to the helicopter was improperly maintained.

News articles dated 2014/06/01.
A Ma Tsu-based Taiwanese fishing boat, months ago, caught an old three-bladed propeller, which is believed to belong to the PBY-5A that disappeared on 1958/10/01.
In that area, two aircraft were lost.
An Albatross shot down in 1966, and the PBY-5A that disappeared on 1958/10/01.
This propeller is made by the USA company Hamilton Standard, and is used on PBY-5A.
On 1958/10/01, it departed Ma Tsu, and disappeared from radars.
Its crew became MIA/BNR.
7th Fleet, USN, searched for 72 hours, but didn’t found any debris.
Also, ROCAF and ROCN searched, but no joy.
Conspiracy theory claimed because October 1 is Chicom National Day, one or more crew or passengers may have attempted to defect to Communist China.
Now the propeller is evidence that the aircraft crashed into sea.
Legally, the propeller is Fu Xing Airways (Trans Asia Airways) property, but TNA isn’t interested.
ROCAF claims it, delivered it to Kao Hsiung (Gao Xiong), and may display it at a ROCAF military history museum.
In California, USA, when TNA founder CHEN Wen Kuan, who’d flown that aircraft, saw a photo of the propeller, his first comment was: why aren’t the propeller blades bent?
That is, when the aircraft crashed, this propeller had stopped.
Pilots were SUN Ming Yuan and CHEN Wei Wen.
Communications operator was LUO Zhao Ming.
Flight engineer was YAN Hui Qun.
Sun, Chen, Luo, and Yan had 20+ years experience, and frequently flew this course, so human error was improbable.
Passengers were three Taiwanese and four Americans.
Passengers included Lian Jiang County mayor WANG Xu, and Ma Tsu Defense Command deputy chief of staff NIU Ying.
It departed Ma Tsu at 5:45 PM.
At 6:10 PM, Luo advised Taipei: “Height, 1,000′; distance from Taipei, 80 NM.”
Afterwards, aircraft disappeared.
One theory was immediate destruction, such as a bomb, and the comm operator didn’t have time to report, but an explosion would cause debris.
Another theory was electrical system malfunction, and disabled the radio, but the aircraft could fly on.
Once, Chen and Sun had this problem, and they could RTB.
After the accident, Ma Tsu radar officer claimed after the aircraft took off, another blip appeared behind it, chased it, and both blips disappeared, as if a mid-air collision, but would cause debris.
Once, one or more Chicom MiG-15s attacked this aircraft, and about 100 rounds damaged its wings, but it could RTB.
Cause of crash is still unsolved mystery.
