In all versions except the interdictor/strike B(I).8, where the nav was expected to bail out of the side entrance door, there was a hydraulic wind deflector ahead of the door to help a little.

My B.2. (built early 50s) is a 3 seater with 3 ejection seats, so yes to that.

Lightnings in service had a pretty good abandonment record and some ejections were survived although right on the limit (and beyond).
Maintained correctly the seat did not fail. If there was a problem escaping, pilots simply would not have flown in them.
This recent fatallity was proven negligence.
A similar incident back in 1962 with a properly serviced seat.
